USMLE vs. PLAB
Choosing between USMLE vs. PLAB is one of the most important decisions for Indian medical graduates who plan to build a medical career abroad. Both pathways are designed for doctors who want to practise in another country, but they lead to different healthcare systems, have different examination structures, and require different preparation strategies.
After passing both parts and meeting the other requirements, an eligible international medical graduate can apply for registration with a licence to practise in the UK.
USMLE vs. PLAB: Key Differences
Feature | USMLE | PLAB |
Primary destination | United States | United Kingdom |
Main regulator/pathway | USMLE/ECFMG and US state licensing system | General Medical Council (GMC) |
Main examination structure | Step 1, Step 2 CK and Step 3 | PLAB 1 and PLAB 2 |
Step 1/PLAB 1 focus | Basic sciences | Applied medical knowledge |
Clinical assessment | Step 2 CK is computer-based | PLAB 2 includes practical clinical stations |
English requirement | Part of the overall IMG pathway and certification requirements | English evidence is required before PLAB |
Post-exam pathway | Residency and state licensure requirements | GMC registration and UK medical employment/training |
Best suited for | Doctors targeting US residency and practice | Doctors targeting UK registration and practice |
The examination structures are different enough that candidates should not choose a pathway simply because one exam appears shorter or easier.
USMLE vs. PLAB Exam Pattern
USMLE Step 1
USMLE Step 1 assesses understanding and application of basic medical sciences to the practice of medicine.
From May 14, 2026, Step 1 uses 14 blocks of 30 minutes each, within an eight-hour testing session. Each block can contain up to 20 questions, with a total of up to 280 items.
Step 1 is currently reported as pass/fail, making the preparation strategy different from exams where the numerical score is a primary outcome.
USMLE Step 2 CK
Step 2 CK focuses on clinical knowledge and the ability to apply medical knowledge to patient care.
From May 7, 2026, Step 2 CK uses 16 blocks of 30 minutes during a nine-hour testing session, with up to 318 questions overall.
