The USMLE (United States Medical Licensing Examination) is the examination pathway associated with medical licensure in the United States, while PLAB (Professional and Linguistic Assessments Board) is a route used by many international medical graduates seeking registration to practise medicine in the United Kingdom. The better option depends on your preferred country, career goals, specialty interests, finances, and willingness to complete the required licensing and training pathway.
What Is USMLE?
The USMLE is a three-step examination program used as part of the pathway toward medical licensure in the United States.
For international medical graduates, Step 1 and Step 2 CK are important components of the ECFMG Certification examination requirements. Step 1 focuses primarily on foundational medical sciences and their application to medicine, while Step 2 CK evaluates the application of clinical knowledge to patient care.
Step 3 assesses whether a physician can apply biomedical and clinical knowledge to independent medical practice. Eligibility for Step 3 includes passing Step 1 and Step 2 CK and meeting the applicable ECFMG and other eligibility requirements.
For international medical graduates applying for USMLE exams from January 2026 onward, USMLE service functions for IMGs transitioned to FSMB. However, ECFMG continues to determine eligibility for ECFMG Certification.
What Is PLAB?
PLAB is the Professional and Linguistic Assessments Board test administered by the General Medical Council (GMC) for doctors who qualified outside the UK and certain other jurisdictions.
The PLAB pathway has two parts:
- PLAB 1: A written multiple-choice examination
- PLAB 2: A practical clinical and communication skills examination
According to the GMC, PLAB 1 consists of 180 single-best-answer questions completed within three hours. PLAB 2 is an OSCE-style assessment involving 16 scenarios, with each scenario lasting eight minutes.
