Why Anxiety Can Increase After Cancer Treatment Ends
Learn why anxiety can increase after cancer treatment ends, including fear of recurrence, follow-up scans, late effects, and survivorship challenges.
Finishing cancer treatment is often seen as a major milestone. Yet, some patients experience more anxiety after treatment ends. The regular structure of hospital visits suddenly changes. Contact with the oncology team may become less frequent. Concerns about recurrence, follow-up scans, late effects, and returning to daily life can become stronger.
Anxiety that emerges or persists after treatment is an important survivorship concern. Research shows that fear of cancer recurrence affects a considerable proportion of cancer survivors. Understanding these emotional changes can help healthcare professionals identify distress early and provide appropriate psychological and survivorship support.
Key takeaways
● Post-treatment anxiety can be linked to several survivorship challenges.
● Fear of cancer recurrence is a major source of distress.
● Reduced contact with healthcare teams can increase uncertainty.
● Follow-up scans may trigger significant anxiety.
● Physical symptoms can be mistaken for signs of recurrence.
● Survivorship care should include emotional assessment.
Why anxiety may increase after treatment
Anxiety may increase during survivorship because the structured medical environment of active cancer treatment changes after therapy ends. During treatment, frequent consultations, investigations, and treatment sessions provide regular clinical monitoring.
After treatment, follow-up visits may become less frequent. This can increase uncertainty about recurrence, future health, and changes in symptoms. The reduction in regular contact with the oncology team may also make anxiety more noticeable.
Therefore, successful completion of cancer treatment does not necessarily mean that anxiety resolves. Anxiety can persist or emerge during survivorship and should be recognised as part of the cancer journey.
Fear of cancer recurrence is a major factor
One of the strongest concerns after treatment is the possibility that cancer could return.
An individual-participant-data meta-analysis included 9,311 patients and survivors. Of these, 58.8% scored at least 13 on the Fear of Cancer Recurrence Inventory–Short Form, while 19.2% scored at least 22, a more stringent threshold. These are questionnaire thresholds, not percentages with a diagnosed psychiatric disorder.
Fear can become stronger before:
● Follow-up scans
● Blood tests
● Oncology appointments
● Surveillance imaging
● Investigations for new symptoms
This recurring pattern is sometimes called scanxiety.
