At present, there are no pharmacological therapies specifically approved for vascular dementia. Consequently, treatment generally focuses on controlling vascular risk factors, managing symptoms, preserving cognitive abilities, and reducing the likelihood of additional vascular events.

Antihypertensive medications, anticoagulants, antidiabetic therapies, antiplatelet agents, vasodilators, antidepressants, antipsychotics, and other supportive treatments may be incorporated into patient management depending on individual needs.

Apabetalone is a selective bromodomain and extraterminal protein inhibitor designed to influence gene expression through epigenetic regulation. Its multi-target mechanism could provide a different approach from conventional treatments that focus primarily on individual disease pathways.

BAC is an investigational anti-inflammatory therapy developed from a Multi-Glycan Complex derived from soybean extract. It is being studied for its potential to address neuroinflammation associated with vascular dementia.

PNA1, developed by ProNeurogen, is an investigational native angiotensin-(1-7) peptide that selectively targets the MAS receptor. The therapy is being explored across vascular cognitive impairment and dementia, traumatic brain injury, and chronic bone cancer pain.

Rifaximin Soluble Solid Dispersion is another candidate being evaluated for vascular dementia. Earlier clinical research has indicated a favorable safety and tolerability profile, alongside changes in systemic inflammation and dementia-associated biomarkers. Its potential mechanism involves modulation of gut microbiota, an area attracting increasing interest in neurodegenerative and cognitive disorders.
