What Is the USMLE Step 2 Exam?
The USMLE Step 2 CK is a computer-based examination that assesses clinical knowledge and its application to patient care.
The exam focuses on whether candidates can use medical knowledge in realistic clinical situations. It also includes concepts related to health promotion, disease prevention, and patient-centred care.
For international medical graduates, passing Step 1 and Step 2 CK remains part of the medical science examination requirement for ECFMG Certification. ECFMG continues to determine certification eligibility, while USMLE exam services for IMGs are now managed by FSMB.
USMLE Step 2 CK Exam Pattern 2026
The Step 2 CK format was updated in 2026. Candidates taking the examination on or after May 7, 2026, take the exam using the new test-delivery software.
The current format includes:
- 16 blocks
- 30 minutes per block
- Up to 20 questions per block
- Up to 318 questions overall
- One-day examination
- Approximately 9 hours of total testing-session time
- A minimum of 55 minutes of break time
- A 5-minute optional tutorial
Although the number of blocks has increased and each block is shorter, the overall examination duration and maximum number of items have not changed.
This change makes time management particularly important. Students need to become comfortable answering questions within 30-minute blocks rather than relying only on the older 60-minute block format.
