Liver metastases are usually found through imaging rather than symptoms. CT is typically the first test used, and MRI may be added when a lesion needs closer characterization or findings are unclear.

• Blood tests, including liver function tests, provide supporting information but cannot independently confirm metastatic spread

• Not every liver lesion found on imaging represents metastatic cancer; some are benign findings such as cysts or hemangiomas

• A biopsy of a liver lesion may be considered when imaging cannot establish the diagnosis with enough confidence, or when tissue confirmation would change management

Imaging findings are interpreted alongside the overall clinical picture, with radiology and oncology teams typically reviewing liver findings together.

Liver metastases represent distant spread and are classified as metastatic, or stage IV, disease. Staging describes how far the disease has spread, but it is a starting point for discussion, not something that alone determines an individual's treatment plan.

Whether the liver is the only site involved, or disease is present elsewhere too, shapes the conversation that follows. Disease limited to a few liver metastases is sometimes called oligometastatic, a pattern that can affect which options are discussed without changing the stage classification.

Treatment for pancreatic cancer with liver metastases typically focuses on controlling the disease throughout the body, relieving symptoms, and maintaining quality of life.

Systemic Treatment for Pancreatic Cancer With Liver Metastases

Because the cancer has already spread beyond the pancreas, treatment reaching the whole body is central to management. Systemic treatment is selected based on overall health, organ function and treatment goals, and adjusted based on disease response.

Liver-Directed Treatment for Selected Metastases

In carefully selected patients, treatment directed at liver lesions is sometimes considered alongside systemic therapy:

• Surgical removal of liver lesions, known as metastasectomy, considered in a small subset of carefully selected patients with limited disease

• Thermal ablation, which destroys liver tumor tissue using heat

• Stereotactic body radiotherapy (SBRT), precisely targeted radiation aimed at individual liver lesions

These local approaches are not routine for every patient with liver metastases. The supporting evidence is largely retrospective and more limited than for systemic treatment, so they are reserved for carefully selected patients based on treatment response, disease distribution and technical feasibility.
