Can a Hernia Become Dangerous? Warning Signs You Should Know

A hernia may not always cause serious problems at first. Some people notice only a small bulge or mild discomfort, while others may experience pain during coughing, lifting, or physical activity. However, a hernia can become dangerous when the tissue trapped in the hernia sac becomes stuck, or its blood supply is reduced. Recognizing the warning signs early is important because complications may require urgent treatment.

When Can a Hernia Become Dangerous?

One of the main concerns with a hernia is that the protruding tissue can become trapped and unable to move back into its normal position. This is known as incarceration. If the blood supply to the trapped tissue becomes compromised, it can lead to strangulation.

A strangulated hernia is a medical emergency because reduced blood flow can cause serious tissue damage. If part of the intestine is involved, prolonged obstruction or loss of blood supply can result in severe complications.

Dr. Anubhav Sangwan, General Surgeon at Miracles Healthcare, Gurgaon, says, “A painful, hard, or suddenly changing hernia should not be ignored, as timely evaluation can help prevent serious complications.”

This is why a hernia that suddenly changes in appearance, becomes painful, or is associated with digestive symptoms should not be ignored.

Warning Signs of a Strangulated Hernia

1. Sudden or Increasing Pain: Pain that suddenly develops or becomes significantly worse can be an important warning sign. If a previously painless hernia becomes extremely painful or remains painful even while resting, prompt medical evaluation is necessary. Pain may occur because tissue has become trapped or because pressure is affecting the surrounding structures.
2. A Bulge Becomes Hard or Tender: A hernia that was previously soft may become firm, swollen, or tender to the touch when complications develop. You may also notice that the bulge no longer changes when you lie down or cannot be gently pushed back. This can indicate that the tissue has become trapped. Do not repeatedly or forcefully try to push a painful or hard bulge back. Seek medical attention instead.
3. Change in the Colour of the Skin: Pay attention to changes in the skin over the hernia. Red, purple, dark, or unusually discoloured skin can be a warning sign that the blood supply to the trapped tissue may be affected. A painful hernia combined with a visible change in skin colour requires urgent medical assessment. 

Dr. Anubhav Sangwan says, “A sudden change in the colour of the skin over a hernia, especially with pain or tenderness, should be evaluated without delay.”

1. Nausea and Vomiting: Nausea or vomiting alongside severe hernia pain can indicate that a portion of the intestine has become trapped or obstructed. This is particularly concerning when vomiting occurs with abdominal swelling, constipation, or an inability to pass gas.
2. Difficulty Passing Stool or Gas: A complicated hernia can sometimes contribute to intestinal obstruction. If you suddenly cannot pass stool or gas and also experience abdominal pain or swelling, you should seek medical care promptly. These symptoms should not simply be assumed to be constipation, especially if you already have a known hernia.
3. Abdominal Swelling: Increasing abdominal swelling or bloating, particularly when accompanied by pain, vomiting, or difficulty passing stool, can indicate that the intestine is not functioning normally. 

Dr. Anubhav says, “Sudden abdominal swelling along with hernia-related pain or vomiting should be assessed promptly to rule out a serious complication.” When these symptoms appear suddenly, medical evaluation should not be delayed.

What Happens if a Strangulated Hernia is Ignored?

A strangulated hernia can restrict blood flow to the trapped tissue. If treatment is delayed, the affected tissue may become severely damaged.

When the intestine is involved, prolonged obstruction and reduced blood supply can lead to serious complications, including tissue death and perforation. In severe cases, infection can spread into the abdominal cavity and potentially become life-threatening.

For this reason, severe pain, vomiting, a hard or irreducible bulge, skin discolouration, fever, or inability to pass stool or gas should be treated as urgent warning signs.

Does Hernia Pain Always Mean an Emergency?

Not necessarily. Hernia-related discomfort can occur with activities such as lifting, coughing, prolonged standing, or physical exertion and does not automatically mean that the hernia is strangulated.

However, new, sudden, severe, or rapidly worsening pain deserves medical attention, particularly if it occurs with other warning signs.

Rather than trying to determine the severity at home, it is safer to have a healthcare professional examine a hernia when symptoms change.

When Should You See a Surgeon?

Consider consulting a general surgeon near you for hernia treatment if you notice:

• A new or enlarging abdominal or groin bulge
• Repeated pain or discomfort
• Pain while coughing, lifting, or exercising
• A hernia that is becoming difficult to push back
• Increasing tenderness around the bulge
• Changes in the size or appearance of the hernia

Emergency medical attention is particularly important when severe pain is accompanied by vomiting, abdominal swelling, fever, skin discolouration, or inability to pass stool or gas.
