Bonsity Expands the Treatment Landscape for High-Risk Osteoporosis
Osteoporosis is a chronic skeletal condition in which reduced bone strength increases the likelihood of fractures, creating a substantial health burden, particularly among aging populations. Treatment strategies for patients at elevated fracture risk continue to evolve as clinicians consider therapies with different mechanisms of action. In October 2019, the U.S. Food and Drug Administration (FDA) approved Bonsity, also known as PF708 or teriparatide injection, for specific osteoporosis populations at high risk of fracture. The regulatory application relied on FDA's established findings for Forteo, the reference product.
FDA Approval Strengthens Bonsity's Role in Osteoporosis
The FDA approval of Bonsity introduced another teriparatide-based option for managing osteoporosis in patients with significant fracture risk. The product is a parathyroid hormone analog containing teriparatide and was developed through the 505(b)(2) regulatory pathway, allowing the applicant to rely partly on FDA's existing findings regarding the safety and effectiveness of the reference product. FDA review documents state that the application demonstrated physicochemical comparability, pharmacokinetic bridging, and comparable immunogenicity between PF708 and Forteo.
The approved indications include treatment of postmenopausal women with osteoporosis who are at high risk of fracture, increasing bone mass in men with primary or hypogonadal osteoporosis who face high fracture risk, and treating men and women with osteoporosis associated with sustained systemic glucocorticoid therapy when fracture risk is high. These indications positioned the product within an established segment of osteoporosis treatment.
Bonsity Injection Offers a Teriparatide-Based Treatment Approach for Osteoporosis
The approved Bonsity injection is administered subcutaneously. FDA labeling specifies a recommended dosage of 20 mcg once daily, with administration into the thigh or abdominal region. The product is supplied through a prefilled delivery device designed to provide daily doses.
Teriparatide works as an analog of human parathyroid hormone and is associated with stimulation of bone formation. This mechanism distinguishes it from therapies that primarily work by slowing bone resorption. FDA documentation describes PF708 as a 34-amino-acid analog corresponding to the N-terminal sequence of human parathyroid hormone.
Treatment duration is also an important consideration. Current labeling states that use of teriparatide for more than two years over a patient's lifetime should generally be considered only when the individual remains at, or returns to, high fracture risk. Consequently, treatment decisions depend on the patient's clinical circumstances and ongoing assessment of fracture risk.
Bonsity vs Forteo Highlights Similarities and Product Differences in Osteoporosis
Interest in Bonsity vs Forteo is largely driven by the fact that both products contain teriparatide and were developed for closely related osteoporosis indications. FDA's review explains that Pfenex submitted PF708 by relying on the agency's previous findings for Forteo. The review also assessed physicochemical comparability, pharmacokinetic characteristics, and immunogenicity as part of the regulatory evaluation.
Although the products share the same active ingredient, they are distinct marketed products. FDA documentation notes that PF708 was manufactured using a strain of Pseudomonas fluorescens, whereas Forteo was manufactured using a strain of Escherichia coli. Therefore, their relationship is best understood through their shared active ingredient, therapeutic purpose, and regulatory pathway rather than treating them as simply the same commercial product.
