How Should Diabetic Shoes Fit?

A properly fitted diabetic shoe should feel comfortable without needing a long “break-in” period.

There should be enough space in front of the longest toe. Diabetes Foot Australia recommends an internal shoe length approximately 1–2 cm longer than the standing foot length.

The shoe should also:

  • Allow the toes to move freely
  • Avoid pressure on the sides of the foot
  • Avoid rubbing against the heel
  • Keep the heel securely in place
  • Accommodate the widest part of the foot
  • Leave sufficient vertical space above the toes
  • Hold the foot securely without excessive compression

Don't assume that going up one size will make a narrow shoe suitable for wide feet. A longer shoe can cause the foot to slide forward, potentially creating additional friction.

How to Test New Shoes

When trying on shoes, wear the socks you normally use with them. Stand and walk around rather than evaluating the shoes while sitting.

Check for pressure around the:

  • Little toe
  • Big toe
  • Ball of the foot
  • Bunions
  • Top of the toes
  • Heel
  • Sides of the forefoot

After wearing new shoes, inspect your feet for redness, blisters, abrasions, or other signs of pressure. Daily foot inspection is particularly important for people with diabetes.

If a particular area repeatedly becomes red or irritated, the shoe may not fit correctly.
