Are Semaglutide Troches Better Than Weekly Injections? Explore Weight Management Options with A2Z RX
4. Blood Sugar Regulation
Semaglutide stimulates insulin secretion in a glucose-dependent manner and reduces glucagon secretion. These actions help regulate blood glucose levels.
The medication is not a substitute for a balanced diet, regular physical activity or appropriate medical care. Its role in weight management depends on the patient's individual circumstances and the specific formulation prescribed.
How Do Weekly Semaglutide Injections Work?
Weekly semaglutide injections deliver medication under the skin using a prefilled injection device. The FDA-approved weight-management formulation is prescribed alongside a reduced-calorie diet and increased physical activity for eligible patients.
The treatment generally involves gradual dose escalation under medical supervision. This approach helps patients adjust to the medication and may reduce gastrointestinal side effects.
Clinical evidence supports the effectiveness of approved injectable semaglutide for chronic weight management.
In the landmark STEP 1 clinical trial, published in the New England Journal of Medicine, 1,961 adults with overweight or obesity without diabetes received once-weekly semaglutide 2.4 mg or placebo, alongside lifestyle intervention.
After 68 weeks, the semaglutide group experienced an average body-weight reduction of approximately 14.9%, compared with 2.4% in the placebo group.
These findings demonstrate the potential effectiveness of the studied injectable formulation. They should not be used to predict the results of compounded troches or other untested formulations.
Are Semaglutide Troches as Effective as Weekly Injections?
This is one of the most important questions for people comparing different administration methods.
Although both formulations may contain semaglutide as their active ingredient, the way a medication is formulated and absorbed can influence how it behaves in the body.
Differences in Absorption
Weekly injections deliver medication subcutaneously, allowing it to enter the bloodstream without relying on absorption through the digestive system.
Troches are designed to dissolve in the mouth. However, the amount of medication absorbed through the oral tissues and the extent of any medication that is swallowed may vary depending on the formulation.
There is insufficient high-quality clinical evidence demonstrating that compounded sublingual formulations provide predictable semaglutide exposure equivalent to approved injectable products.
Differences in Clinical Evidence
Approved injectable semaglutide has been studied in large randomized clinical trials involving adults with overweight or obesity.
By comparison, compounded troches do not have an equivalent body of clinical evidence establishing their weight-loss outcomes, optimal dosing or long-term safety.
An Important Difference: FDA-Approved Oral Semaglutide
Patients should also understand that a compounded troche is not the same as an FDA-approved oral semaglutide tablet.
In December 2025, the FDA approved the once-daily Wegovy oral tablet for chronic weight management in eligible adults. This formulation has its own clinical evidence, approved dosing instructions and safety information.
Therefore, the comparison is not simply between injections and all oral formulations. The specific product and its regulatory status matter.
