Common Causes of Gynecomastia

Hormonal changes are one of the main causes of gynecomastia. These changes may happen naturally during puberty, adulthood, or older age.

Teenage boys may develop temporary breast swelling during puberty. In many cases, it becomes smaller as hormone levels settle. Adults may develop the condition because of age-related hormonal changes, medicines, health conditions, or substance use.

Possible causes include:

  • Natural hormonal changes during puberty
  • Age-related reduction in testosterone
  • Certain medicines
  • Anabolic steroid use
  • Significant weight gain
  • Liver, kidney, thyroid, or hormonal problems
  • Certain testicular conditions
  • Heavy alcohol or recreational drug use

In some cases, the exact cause cannot be identified. This is known as idiopathic gynecomastia.

Patients should not stop taking prescribed medicine without medical advice. A doctor can review the medicine and decide whether a safe alternative is available.

Signs and Symptoms of Gynecomastia

The main sign is enlargement of one or both sides of the male chest. The size and shape can be different for every patient.

Common symptoms include a firm area under the nipple, puffy nipples, chest tenderness, uneven breast size, and discomfort during movement or exercise.

Some patients do not experience pain. Their main concern may be the appearance of the chest under fitted clothing. Others may avoid swimming, sports, changing rooms, or social activities because they feel self-conscious.

Certain symptoms require faster medical attention. These include a hard fixed lump, nipple discharge, bleeding, skin dimpling, rapid enlargement, severe pain, or swelling in the armpit. These symptoms do not automatically mean a serious disease, but they should not be ignored.
